Preparation for Adulthood Vulnerable Adult Team – now live

I’m delighted to let you know that as part of the Preparation for Adulthood Service, the Vulnerable Adult Team is now able to take connection requests and start supporting young people and younger adults who are aged 14 – 30 years old and require support in any of the following four areas –
- Health (both physical and mental)
- Employment
- Housing
- Friendships, Relationships and Community
The Vulnerable Adult Team can support young people who
- Have endured trauma and/or have mental health difficulties either diagnosed or undiagnosed
- Have a vulnerability that could pose a risk or cause harm to themselves or others
- Are at risk of offending
- Are at risk of being exploited
- Are currently a part of the criminal justice system (CJS) as an adult or young person either in custody or in the community
- Are leaving the CJS or a statutory service who require support in our four key areas where it will no longer be provided
- Are experiencing homelessness or are at risk of homelessness
- Need support in at least one of our four key areas and meet the above criteria—health, housing, employment, friends, relationships and community

In order to understand how we can help a young person / younger adult, we would like to know what they would like to achieve /what support they would like in any of the four key areas. To help collect this information you can complete an Outcome Engagement Tool to support this conversation located on NWG resources search Preparation for Adulthood Vulnerable Adult Team.
We will work alongside your service to support young people / younger adults to work with us in the initial stages of contact to make sure they understand how we can help, so we welcome conversations to talk about suitability and if a joint approach may help with motivation and engagement.
